Logic Model Template
What is a logic model?
- A graphic representation or "blueprint" of the key elements of a program or project, and how it will work under certain conditions to "solve" identified problems.
- A helpful tool for identifying outcomes and depicting the "chain of events" that link inputs and activities to outputs and outcomes.
- A vehicle for dialogue, planning, project improvement and evaluation.
